Routine
appointments
are generally
scheduled
several
weeks in
advance.
Urgent needs
are met
within one
to two days
and true
emergencies
are seen
immediately.
It may be
necessary
for you
to see someone
other than
your usual
physician
if your
need is
emergent. |

Please
arrive 15
minutes
before your
scheduled
appointment
to complete
registration. |
|
|
If you
are unable
to keep
your appointment,
please notify
our office
24 hours
in advance.
You may
be charged
a fee of
$25 for
failure
to cancel
an appointment
without
appropriate
notice. |

|
In an
effort to
stay on
schedule,
we reserve
the right
to reschedule
your appointment
if you are
more than
15 minutes
late. |

Prescription
refill
requests
are
accepted
by phone
or
through
our
convenient
online
message
service.
After
physician
approval,
refills
will be
processed
within
one
business
day. |

|
A
physician
from CWH
is
available
for
emergencies
24 hours
a day.
Nonemergent
concerns
or
questions
may be
left on
our
after-hours
voicemail
system
at
803.254.3230
or by
using
our
online
message
service.
You will
receive
a
response
the
following
business
day. |

All
medical
records
are
confidential.
No
information
is given
over the
telephone
without
your
written
consent.
Your
written
authorization
is
required
before
records
will be
released
to
insurance
companies
or
others.
There is
a
minimum
charge
of
$15.00
for
copying
and
mailing
records. |

|
Payment
for
services
is
expected
at the
time of
your
visit.
This
includes
co-pays,
deductibles,
and
coinsurance.
As a
convenience
for you,
we
accept
major
credit
cards,
cash and
checks. |
|
|
Based on
your
health
plan,
CWH will
attempt
to
determine
the
appropriate
lab or
facility
for
studies
performed
outside
our
office.
However,
it is
ultimately
your
responsibility
to
notify
us of
any lab
or
facility
that has
preferred
status
with
your
health
plan. |
